We are proud to be the only primary care clinic on the Eastside offering early morning, late evening, and weekend appointments. Same-Day Appointments

Life doesn’t always follow a schedule—and we understand that. That’s why we reserve slots each day specifically for same-day appointments. If you don’t see availability online, just give us a call at 425-300-WELL (9355). We’ll do everything we can to get you seen.

Home Visits for Eligible Patients

For our elderly patients or those experiencing serious illness, traveling to the clinic may not be possible. In these cases, home visits may be available. Please contact us to see if a home visit can be arranged. Requests are considered on a case-by-case basis and are subject to provider approval.

Fill out your New Patient paperwork in advance via our Patient Portal, or arrive early
to complete it in person before your visit.

What to Bring to your First Visit

✅ Valid Insurance Card
✅ Government-issued photo ID
✅ Your medical history (printout from last Primary Care visit, if available)
✅ All current medications (prescription & over-the-counter, in original containers)
✅ A valid form of payment (credit card, cash, or check)

Arrival

Please arrive 15 minutes before your scheduled appointment. Late arrivals may need
to be rescheduled.

Note For New Patients

We’re excited to welcome you to our practice! To ensure you receive the best care possible, your first visit will be a New Patient Visit. This appointment is
focused on establishing care, reviewing your medical history, and addressing any current health concerns.

The New Patient Visit helps us get a comprehensive understanding of your health, while the Wellness Exam is designed for preventive screenings and routine health maintenance. Separating these visits allows us to dedicate appropriate time to each and helps ensure full insurance coverage.

After your initial visit, we’ll be happy to schedule your follow up visits, Annual Wellness Exams and more. We look forward to supporting you on your health journey!

Our Policies:

Late Arrival Policy

Patients who arrive more than 15 minutes after their scheduled appointment time may be asked to reschedule. This helps us respect the time of all our patients and maintain the flow of care.

Cancellation Policy

If you need to cancel or reschedule an appointment, please provide at least 24 hours’ notice.
Appointments changed or canceled with less than 24 hours will incur a fee.

Female Patient Examinations

For any hands-on physical examinations involving female patients, a female staff member will be present in the room. This is a standard practice intended to ensure patient comfort and uphold the highest standards of professionalism and care.
