WHOLE-PERSON PRIMARY CARE

Diabetes Care That Connects the Whole Picture

Blood sugar is only one part of diabetes care. Effective management also considers blood pressure, cholesterol, kidney health, weight, medications, cardiovascular risk, sleep, nutrition, physical activity, and complications that can develop over time.

At Hemlock Clinic, Manpreet Sran, M.D. and the care team use a primary care approach that brings these areas together. When possible, you continue seeing the same provider over time.

TESTING TRIGGERS

When to Ask About Diabetes or Prediabetes Testing

Diabetes and prediabetes may be identified through routine screening before noticeable symptoms appear. Evaluation may also be appropriate with increased thirst or urination, unexplained weight changes, fatigue, blurred vision, recurrent infections, obesity, family history, high blood pressure, abnormal cholesterol, or another metabolic risk factor.

The A1C test can help diagnose prediabetes or diabetes and shows average blood sugar over roughly the previous two to three months. A1C and other laboratory tests are ordered through outside laboratories, commonly Labcorp or Quest.

YOUR FIRST VISIT

A Clear and Supportive Process

Your first visit generally lasts about 30 to 40 minutes. The clinician reviews your symptoms, prior glucose and A1C results, medications, family history, nutrition, physical activity, weight, blood pressure, cardiovascular risk, and possible diabetes-related complications.

Bring your medication list or medication containers, recent laboratory results, home glucose readings, CGM reports if available, insurance information, and questions you want to discuss. Based on the evaluation, the clinician may order additional tests and create a follow-up plan.

DIABETES AND A1C TESTING

Comprehensive Testing for Informed Care

Hemlock Clinic does not perform A1C testing onsite. Testing is completed through outside laboratories. A1C is commonly checked about every three months when treatment is changing or goals are not being met. Stable patients may be monitored less frequently, often about every six months.

DIABETES MANAGEMENT

Safe, Evidence-Based Options

Medication decisions consider your diagnosis, health history, laboratory results, other conditions, potential benefits, side effects, contraindications, alternatives, and insurance coverage.

Review Your Medication Options

Medication options may include metformin, SGLT2 inhibitors, GLP-1 receptor agonists, DPP-4 inhibitors, sulfonylureas, insulin, or other therapies when clinically appropriate. No medication is suitable for every patient.

Insulin and Continuous Glucose Monitor Support

Insulin and prescription continuous glucose monitors can be managed when appropriate. Complex insulin needs may require endocrinology. Insurance coverage cannot be guaranteed.

MONITORING AND PREVENTION

Preventing and Monitoring Diabetes Complications

Ongoing diabetes care looks beyond blood sugar. A1C and glucose goals are individualized according to age, duration of diabetes, hypoglycemia risk, medications, cardiovascular or kidney disease, other conditions, overall health, and patient preferences.

NEW DIAGNOSIS SUPPORT

Support After a New Diagnosis

A new diagnosis can bring a long list of questions. Hemlock Clinic explains what your results mean, reviews nutrition and physical activity, discusses medication options, addresses monitoring when needed, and creates a clear follow-up plan. When Specialist Care Is Needed

Referral or co-management may be recommended for type 1 diabetes, gestational diabetes, recurrent severe low blood sugar, difficult-to-control glucose, complex insulin therapy, or suspected advanced complications.
