Required Laboratory Testing

Performed at a laboratory and may include testing for:

  • Gonorrhea

Testing requirements vary based on age and USCIS guidelines.

Required Vaccinations

If any vaccinations are missing, they can be completed at a licensed pharmacy.
Please do not receive additional vaccines before your appointment. Our Civil Surgeon will determine exactly which vaccines, if any, are required.

What to Expect

We prefer to see all family members attending for immigration exams during the same
appointment whenever possible.
Only one formal clinic appointment is typically required. Remaining steps can usually
be completed on a flexible drop-in basis.

Children are only required to attend the first appointment.

Expedited completion may be available for urgent USCIS submissions. Eligibility depends on:

Once completed, each certified I-693 form will be sealed in an official USCIS envelope for submission to your attorney or directly to USCIS. A digital copy will also be provided for your records.

The doctor determines which age-appropriate vaccinations are required. Vaccines are completed at a pharmacy because Hemlock Clinic does not administer them onsite.

Form I-693 completion

After required results and documentation are received, Dr. Sran completes the form. You receive the original in a sealed envelope for USCIS or your attorney, plus a copy for your records.

  • Valid government-issued photo ID, such as a passport, driver’s license, or state ID.
  • Original vaccination records from the United States or another country. Clear, printed copies are helpful.
  • Any previous Form I-693 if this is a repeat or corrected filing.
  • Relevant medical documentation requested by the clinic after scheduling.
  • Information for every family member being scheduled: full legal name, date of birth, phone number, email address, and sex.

Do not get extra vaccinations before the appointment solely for this exam. Bring the records you have, even if they are incomplete or from another country. The civil surgeon will review them and explain what is still required.

  • Separate: laboratory charges, vaccinations, imaging, treatment, or additional evaluation that may be needed based on results.
